New Patients
Patients who are new to Westport Eyecare Associates or who have not visited our office in more than a year should arrive 15 minutes prior to their scheduled appointment time to complete their registration.

For your convenience, the registration form and medical history form can be printed. Please complete and bring to your appointment.
Please also bring the following to your appointment:
- Medical and vision insurance card(s)
- List of current medications and medical conditions
- List of other physicians previously or currently involved in your care, as well as any relevant referral information and prior records (if possible)
